Parents and Student/Athletes

You must have a current Sports Physical (EL2) and Parent Permission (EL3) on file with the SGMS Athletic Department in order to participate in tryouts. The physical must be completed on the F.H.S.A.A. forms - EL2 and EL3

All non-traditional students (Home-Schooled, Charter School, Private School) must also have a completed EL7 verified by the I.R.C.S.D., in order to participate in tryouts.
