Parents and Student/Athletes

You must have a current Sports Physical (EL2) and Parent Permission (EL3) on file with the SGMS Athletic Department in order to participate in tryouts. The physical must be completed on the F.H.S.A.A. forms - EL2 and EL3

All non-traditional students (Home-Schooled, Charter School, Private School) must also have a completed GA7 verified by the I.R.C.S.D., in order to participate in tryouts.

Admission Fee: Most middle schools will be charging to watch the athletic contests this year for all sports. We have decided to charge this year at our games as well. The cost will be as follows:

• Adults - $3.00
• Students/Children - $2.00

Storm Grove is a proud member of the FHSAA. We are governed by them and must follow all their regulations and by-laws. All students/parents will need to complete and have on file the following FHSAA forms in order to participate in tryouts:

Once a student has successfully made a team, parents must attend a Athlete/Parent meeting to:

• Complete the Student Athlete/Parent Contract
• Complete the SGMS Player Parent Package
• Pay the $85.00 Pay to Participate Fee - (for Fall and Winter Sports) - Soccer, Volleyball, Basketball, Cheerleading
• Pay the $85.00 Pay to Participate Fee - (for Spring Sports) - Baseball, Softball, Lacrosse
